Live voice dictation and stop-and-send

• The recording pill has a send button that stops dictation and sends the prompt once the last words are in.
• Stop dictation and send (⌘⌥↵ by default, rebindable in Settings → Keybindings) works wherever focus is.

Improved

• OpenAI Realtime dictation transcribes at each pause instead of only after you stop, so stopping waits only for the last sentence.
• Silence auto-stop adapts to background noise, ignores clicks and key taps, and keeps working when the window is in the background.
• Cleaner audio reaches the transcription model, and audio streaming uses far less overhead.

Fixed

• Stopping right after silence no longer shows an error or inserts a made-up "Thank you."
• OpenAI Audio and local whisper no longer cut off your last words or insert the text twice.
