Live status that names the work, not a frozen "Thinking…"
While the agent is working, chat shows what it is doing — editing a file, running a command — instead of a blank turn or a generic "Thinking…" covering the real work.
Fixed
- An in-flight step with no prose yet no longer looks frozen; the live status line still has a home.
- Streaming "Thinking…" no longer hides a status that names concrete work (Editing a file, Running a command).
- A malformed plan envelope no longer wipes the rest of the assistant message after the turn settles.
- The live plan card and the inline plan no longer vanish together while the envelope is still streaming.
- A review patch that fails to apply is kept and logged instead of disappearing.

- Live status can name the tool being composed before it starts, without drawing a ghost tool row.
