Code review you can trust

New

• Reviews run as read-only review agents, one per group of files, so large pull requests are no longer cut off at a fixed size.
• A second pass tries to disprove each finding. Disproved findings are removed; the rest are marked Verified or Unconfirmed.
• Every finding shows the evidence behind it, and the review says which files it read and which it skipped (lockfiles, generated files) and why.
• A pull request that is not checked out is reviewed against its own code in a temporary checkout, leaving yours untouched.

Fixed

• VORTEX_REVIEW_DIFF_CHAR_CAP, VORTEX_REVIEW_MAX_COMMENTS and VORTEX_REVIEW_PROVIDER now take effect.
• vortex review compares against origin/HEAD like the app, and --fail-on counts only the findings at or above the threshold.

Changed

• Reviews take longer and cost more than before. Set VORTEX_REVIEW_MODE=one_shot for the previous single-pass review.
